My name is Karen Cheah, mother to 4 boys, and community pharmacy owner in Margaret River, Western Australia since 2006.
I formulated and founded DermaGen by Botanical Chemist™ after seeing many patients in my community suffering from skin conditions such as eczema, psoriasis, sun-damaged skin, acne, pimples and rosacea.

Topical Steroids aren't always the answer.

As a mother and as a pharmacist, I know topical steroids aren’t always an answer to skin issues in the long run.
My own son had a really bad experience with eczema so I know first-hand how difficult it is to find products that work and deal with the symptoms effectively.

Once we installed a compounding room, I was able to test different formulations with my customers to find out what ingredients gave the best results. DermaGen by Botanical Chemist™ was created after a few years of research and trials of different ingredients.

The benefits are numerous for patients with these skin conditions, in particular patients who are having sore, inflamed skin after treatment with Efudix, diabetes patient's with sores that heal very slowly, and babies and young children with eczema/dermatitis rashes who may benefit from an alternative to topical steroids.

Topical Steroid Withdrawal (TSW)

TSW is controversial and the diagnosis can be problematic. It is often difficult to identify whether a flare-up observed following cessation of topical steroid use is due to a worsening of the skin condition, or rebound phenomenon after prolonged, daily application of the topical steroid.(1)

There are features of TSW that are not conventional features of Atopic Dermatitis (AD) such as widespread red skin and itchy, burning pain, excessive skin exfoliation, oedema, skin sensitivity, ‘red sleeve’ and/or ‘elephant wrinkles’.

Potential side-effects of prolonged topical steroid use.

• Skin thinning, • off-colour stretch marks, • rosacea, • perioral dermatitis and • acne.(2)

There is less fibroblast growth and collagen synthesis. The effects are reversible when the topical steroids have been stopped, but it may take months or years for the skin to normalise again, depending on factors such as duration, potency, site of application and surface area covered.

There is a greater increase in other skin infections such as fungal,bacterial and viral(2) and wound healing is impaired.(3) Tolerance or desensitisation can occur as the tissue becomes less sensitive to the steroid, with increasingly potent steroids required to achieve the same results. A rebound effect can also happen on withdrawal of potent topical steroids, as the steroid causes vasoconstriction, followed by a rebound dilation when stopped, and made worse by the atrophy of the epidermal layer.(2) A high absorption of topical steroids may also lead to effects on the adrenal system.(2)

Factors that increase absorption and side-effects of topical steroids:

  • • Longer duration of use and higher potency of steroid cream
  • • Occlusion following application
  • • Application on broken, raw/inflamed skin
  • • How thin the skin is ie around the eyes and genital areas
  • • How much surface area is covered

Factor’s that can contribute to TSW:

  • • Using potent steroids for a long period
  • • Application on skin that is thinner ie eyelids, face, genital areas, as this increases
  • • On young babies/children as their body mass to volume ratio is smaller

Before discontinuing topical steroids, symptoms are:

  • • A ‘rebound’ redness between applications
  • • A rash that spreads and develops in new areas of the body
  • • An intense itching, burning or stinging sensation
  • • A failure to clear with usual course of treatment, requiring increasingly potent preparations to achieve comparable effects
  • •  Increased general allergic responses

After discontinuing topical steroids, symptoms are:

  • • Skin flushing bright red, resembling a sunburn
  • • Visible and measurable flaking of skin – appears to be ‘snowing’
  • • Oozing exudate from lesions
  • • Skin cycling between oozing, swelling, burning, and flaking
  • •  Red sleeves: (arms/legs become red and inflamed,sparing palms/soles)
  • •  Altered thermoregulation (feeling too cold or too hot)
  • •  Hypersensitivity of the skin to water, movement, moisturiser, fabrics, temperature, etc.
  • •  Nerve pain, sometimes described as “sparklers” or “zingers”
  • •  Enlarged lymph nodes
  • •  Skin atrophy (often manifesting as “elephant wrinkles“)
  • •  Hair loss: (head and/or body)
  • •  Insomnia and altered body clock
  • •  Fatigue
  • •  Emotional fluctuations, depression or anxiety

Topical corticosteroid (TCS) withdrawal (addiction) is a potential complication of TCS treatment, particularly where there has been inappropriate long-term use of moderate-to-potent TCS on the face or genital area. By being aware of TCS addiction and withdrawal,practitioners may be better able to help prevent this adverse effect and offer supportive care to patients in TCS withdrawal.” Refer to the Guidelines by Dr Belinda Sherry for management tips and clinical diagnosis.(3)

TSW is a serious medical condition that requires medical support with the management of the symptoms and potential bacterial infection when the skin is broken. Please consult your GP for medical guidance on how to withdraw safely from potent steroids is you have symptoms of TSW as stopping abruptly can lead to a flare-up. The best way is to gradually reduce the potency of the topical steroid to the lowest effective steroid, and space out the days of application, stopping when the symptoms improve. Always use the smallest amount and the least potent steroid that is required, and taper down gradually to avoid a flare up. It’s not recommended to apply topical steroids on broken skin as this increases absorption, so using something to heal the sores would be a better way to manage the symptoms.

Case study of Jasper - Topical Steroid Withdrawal symptoms

Jasper was 3 months old when he suffered from a bronchial infection and had to have a nebuliser steroid. It was after this that he had an Eczema reaction and started using steroids. As time went on, he had to use stronger steroids on his skin and it was never clear (not even on steroids). He had used it on localised areas such as behind the knees, elbows, and a few spots on his back and tummy, but the withdrawal reaction was all over his entire body, including his face.

I met Jasper’s mum online and her son, now 8, was having one of the worst flare-ups. He had come off topical steroids for 2.5 years. Was already taking a multi-vitamin, Magnesium/Zinc/Calcium, probiotics, drinking Aloe Vera juice and a supplement for his immune. His lymphnodes were swollen, his skin was so sensitive, contact with water would hurt. He had to miss school to rest and stay cool.. He was already using Zinc/Castor oil cream and wet wrapping.

Using the Manuka Oil Cream and Balm gave the fastest healing and recovery from a flare up. 9 Months later when I checked in again, his flare-ups were very minor and manageable with the Manuka Oil Balm. 12 months later, he still has not had any major flare-ups.
